The provision of Proactive and Preventative quality health care as well as our own brand of personal support services is a key to allowing for the greatest measure of personal dignity and independence to our residents.
We know a caring, professional environment is a comfort to both the patient and the family, when a resident is in need of restorative nursing care for post-hospital recovery, temporary care for illnesses and injuries or long term care.
We also know a change in the level of care for any resident is taken very seriously and involves input from the resident, family members and the health care staff. Active Life's health care is designed to allow residents to retain as high a level of personal independence for as long as possible before making a move. When totally independent living becomes difficult, a resident can simply shift to an assisted living level of care.

Wellness Programs
Our Wellness Program is designed to help residents stay well and remain independent for as long as possible.
Active Life's philosophy of health care is based on the concept of a partnership between residents and staff to assure both the optimal level of health care services and the independent functioning of residents. The on-site clinic and the wellness department work hand-in-hand to enable residents to maintain their health and to prevent injuries.
Directed by professional staff, the Wellness Program is the first step in maintaining long-term health. The staff facilitates activities and helps residents develop individual fitness programs designed to meet their specific goals. It works in tandem with our rehabilitation therapy programs and residents to provide balanced fitness programs for all.
In addition, the fully-equipped fitness room, walking programs, and an array of exercise classes and community activities keep Active Life patients young at heart. Our wellness programs encourage staff and residents to interact in activities for fitness, fun and fellowship. We offer four specialized programs:

Proactive Preventative Healthcare
Active Life's goal is to help retain census in the Assisted Living and Independent Living Setting by using early intervention therapy.
The Active Life Rehab Department offers a unique perspective to physical, occupational, and speech-language therapy. We provide a multidisciplinary approach with fully equipped sensory integration and orthopedic gyms. Patients receive evaluations and therapeutic intervention as needed from a team of specialists.
This team approach allows specialists from all disciplines to assess and analyze each patient’s individual needs and map out a customized treatment plan. Benefits to residents, therapists and facility:
• Maintain Census - retaining a resident allows for maximal cost effectiveness.
• Decrease burden of care and worker's compensation prevalence.
• Increase caregiver diligence.
• Decrease patient's risk of falling.
• Increase Physician, resident and family satisfaction.
How are we able to provide for both our facility and its residents?
Active Life will handle the rigors of Medicare compliance for its facilities:
• We will help you meet Title XXII Regulations by identifying medical and physical decline, documenting it, and notifying all parties responsible for reducing the decline.
• Active Life is Medicare Certified - we bill Medicare directly at no cost to the patient.
• We also bill co-insurances and accept certain HMO's.
• We treat only with a Physician order and a signed letter of consent from the resident.
The Rehab Department works hand in hand with the Social Services Director, Director of Nursing, Dietary Care Director, Activities Director, Physicians, Nurses and most importantly family. The Department consists of:
• Rehab Director *
• Physical Therapist
• Occupational Therapist
• Speech Therapist
• Therapy Technician

The Rehab Director, typically one of the rehab team members, oversees the rehab program, attends meetings, assists with admissions, coordinates paperwork, in-services and other vital rehab & facility needs.
Active Life expects to provide the following:
• Facility and family in-services & instruction
• Therapy screenings of facility admits, re-admits, quarterly care plans and nursing/family referrals
• Assistance with management of the Medicare Program
• Participation in facility/community activities
• Pre-admission screening assistance
• Participation in Fall, Restraint, Weight Loss committees
• Rehab/UR meetings
